This form has been designed as the first step in a multi-step process for syndicated condo owners to book an appointment with a certified appraiser.

The form only takes about 10 minutes to fill out, but if you have an difficulties getting through it, just reach out to us and we will help you. The easiest way to contact us it’s just by clicking on the flashing pink button at the bottom hand of your screen.
How to fill out the form
The form is quite straightforward, however, here are a few tips to make sure you get through it.
First, you will be prompted to give us some details about who you are, your Syndicate, insurance company, and Appraisal Company.
On the next page, you’ll be asked to provide details about rooms in the reference. For this section, you need to add rooms with plumbing, such as kitchens and bathrooms. If a room has a sink or a bathtub, you should include it in this section. Once you’ve completed this section, you can move on to the next section by clicking NEXT at the bottom of the form.
In the following section, you’ll be asked to provide details about rooms in the reference unit that contain DO NOT contain any plumbing. For example, a master bedroom or TV room.
Each section starts with one room . To add more, just click the button below the first room. Here’s a screenshot to help you identify the button in case you miss it.
Click on it and add as many rooms as needed to fully describe the reference unit.
Click next again to be taken to the last page of the form. When you get to the end of the form hit SUBMIT. And that’s it, you’re done.
